Alliance was born of the merger of Bayview Community Hospice, founded in 1988, to serve people in North York, and CommunityCARE Home Hospice, founded in 1990, to serve people with HIV/AIDS throughout the City of Toronto.

It all began in 1970, when Circle of Care was launched as a demonstration project called Coordinated Services to Jewish Elderly, under auspices of the Toronto Jewish Welfare Fund (now the UJA Federation of Greater Toronto). We have gone through a few name changes over the years to reflect our expanded mandate. We now serve individuals of many faiths and cultures, and our clients include people of all ages.

Hospice Palliative Care Ontario (HPCO) was formed in April 2011 when the memberships of The Hospice Association of Ontario (HAO) and the Ontario Palliative Care Association (HPCO) voted to join together and create HPCO. The integration of the two associations, which began as an idea discussed by the boards of both associations in 2007/2008, has been welcomed by a wide range of stakeholders in Ontario. The joining of HAO and OPCA was a significant achievement for hospice palliative care in Ontario, unifying the sector's voice to strengthen hospice palliative care for the people of Ontario.
